General introduction

Diabetes is a progressive disease that affects approximately 30,000 people in the Warrington and Halton area.

We provide services at both Warrington and Halton hospitals and our philosophy is to support individuals and health care professionals to manage care safely and effectively in both community (primary) and hospital (secondary) care.

By helping you manage your diabetes we aim to prevent you needing to spend time in hospital. We support individuals to manage diabetes at home / in the community wherever possible.

Our team of diabetes specialist nurses delivers quality education for individuals and other healthcare professionals. We also run clinics for complications caused by diabetes and provide direct inpatient care if you need to be admitted to hospital for treatment.

We provide the following clinics:

  • New patient clinics – following referral to the Diabetes Service, usually by your GP / practice diabetes nurse.
  • Specialty clinics – focused on a particular aspect of diabetes care, such as insulin pump clinic or young person's diabetes clinic
  • Nurse-led clinics – led by the diabetes specialist nurses
  • Dietician-led clinics – specialist diabetes dieticians provide advice and support on particular aspects of diabetes care. 
How to access this service

Most patients are referred to the service from their GP. 

Patients are then offered appointments to clinics dependent on their specific requirements.

Structured education services for patients

All individuals wishing to attend a face-to-face structured education session need to have a referral sent to the Diabetes Service from their GP practice.

We also offer our 'MyDiabetes' education programmes for patients to help you manage your diabetes:

Type 1 diabetes

'My Diabetes' is a two hour session face-to-face group course for people with type 1 diabetes who want to learn how to self-manage their diabetes. People with Type 1 diabetes are taught to match their insulin dose to their chosen food intake on a meal-by-meal basis. 'MyDiabetes' provides structured group education and is delivered by specially trained diabetes specialist nurses and dieticians, to groups of between 10 to14 people. It is an evidence-based, professionally delivered, and evaluated education package for type 1 diabetes.

Type 2 diabetes

'MyDiabetes' is a two-hour face-to-face group course for people with type 2 diabetes who want to learn how to self-manage their diabetes. They are taught to be more aware of the affects of their food choices will have with medications and lifestyle. 'MyDiabetes' provides structured group education and is delivered by specially trained diabetes specialist nurses and dietitians, to groups of between 10 to14 people.
