General introduction

Ward K25 is a general medicine ward for sub-acute patients requiring ongoing treatment for a range of conditions.

K25 staff support patients' discharges by liaising with the Integrated Discharge Team and therapy staff to ensure the most appropriate destination for the next place of care or recovery.

How to access this service

Patients are transferred to Ward K25 from other wards for continuation of their care.
