How to access medical records

Access to medical records can be requested by downloading the Access to Health Records form.

If you require further advice about accessing your personal information or are a legal firm requesting client medical records, the Trust’s Medico-Legal Team can be contacted at: whh.medico.legal@nhs.net.

If you have any further queries on the use of your information please speak to the healthcare professional providing your care or Patient Advice and Liaison Service (PALS).

Should you wish to complain about the use of your information please contact our Complaints Team.
